Chronic Diseases Management

Chronic Diseases Management is a type of long-term care and support that provides people with a chronic health condition with the medical care, expertise, skills, and services they need to help manage their condition on a daily basis. This may include regular visits and assistance from the family physician or other primary care provider, community-based programs, or referrals to specialist programs and facilities.

Chronic disease is defined by the World Health Organization (WHO) as being of long duration, generally slow in progression, and not passed from person to person i.e. heart disease, diabetes, chronic kidney disease, asthma and chronic obstructive pulmonary disease (COPD), depression, and arthritis, among many others.

You will find the current and available solutions for the management of chronic diseases i.e. guidelines, treatment protocols and self management techniques.
